Dear Field Parents and Guardians,
We hope you are having a great week! It was so nice to connect with parents during our first evening of parent-teacher conferences. We look forward to seeing more parents next week on Tuesday and Thursday. Thank you to our PTO for coordinating dinner each evening for staff!
A few reminders:
- Parent teacher conferences are on October 8 and 10. Sign up has now closed. Please reach out to your child's teacher with any questions.
- There is no school on Friday, October 11th and Monday, October 14th.
- Please follow the rules of the road when driving for drop off - our school is in a neighborhood and we want to be sure we are being good neighbors with our parking and driving! We've received calls from neighbors this week who are very frustrated with blocked driveways and have alerted the Park Ridge police.
- Please remember that there is no supervision before or after school on the playground or blacktop. Children should be supervised by a grown-up.
- Please be sure you call the main office as early in the day as possible if your child is not taking the bus home.

Important Health Forms Deadline!
Important Health Deadline for Students in Kindergarten and those new to D64:
The state deadline for submitting proof of vaccination, physical exams, and eye exams is quickly approaching on Tuesday, October 15th. Due to the holiday on October 14th, health forms must be uploaded to Powerschool by 3 pm on Friday, October 11th. The building nurses have sent numerous communications home to those who are currently missing required information or forms.
- Proof of vaccination and a physical exam dated after 8/14/23 is required.
- Please make sure you’ve answered all questions in the parent health history section, signed, and dated it before uploading. Forms are incomplete without this done.
- Students in kindergarten or attending an Illinois school for the first time are required to submit an eye exam from an optometrist/ophthalmologist.
Please review your child’s Forms/Health Office Document Submission section of Powerschool to ensure you’ve uploaded everything needed. If you have not submitted all required forms, please submit your forms ASAP so they can be reviewed by the school nurse and corrected if missing required information before the deadline to prevent exclusion from school. Instructions on uploading forms can be found on the health services website: https://www.d64.org/health-services/
Students not compliant with vaccine/physical exam requirements will be excluded from school beginning October 15th.

Calling all parents with flexible schedules! We are looking for additional adults to help support our lunch program. The pay for lunch is $24 an hour for a 1.5 hour job. Responsibilities include…
Setting up tables and benches
Passing out lunches
Assisting with opening containers and spills
Assisting students with sorting garbage (trash, recycling, compost)
Encouraging kind conversation and appropriate voice volume
Cleaning and stowing tables and benches

Field Green Team Clean Up Event
Join your Field Green Team and other Green Teams from across District 64 on Saturday, October 5th from 1-3 pm for a Woodland and Park Clean-Up Event. We will meet at Belleau Lake (2137 Busse Hwy, Des Plaines). Long pants, hard-sole shoes, hats, and gardening gloves are recommended. Also, bring a 5 gallon bucket (if you have one). Plastic bags & gloves will be provided.

Lunch Information
Children are also permitted to leave school for lunch. Parents need to come to the main office and sign out their child(ren). Students would remain home for the full hour of lunch/recess.
Our PTO sponsors a weekly pizza lunch on Tuesdays. Please be on the lookout for information on ordering your child's weekly pizza soon!

V-Show Newcomers!
V-Show is a parent written and produced show that has been running since 1953! Each year it raises money to fund things like playground equipment, classroom programs and audio visual equipment.
There are big and small ways to get involved on stage, acting, singing or dancing, or off stage with sets, techdeck, advertising or tickets.
