Decision-Making Support Planning for Transition
Preparing for transition to adulthood is challenging, especially for someone with a disability. There are many options and things to consider. One important consideration is how much help the person will need in making life decisions. Do they need a conservatorship or other decision-making supports?
This webinar will provide information about decision-making options like Conservatorship, Durable Power of Attorney, Supported Decision-Making, etc., and how to choose the right supports for your loved one. We will also provide an overview of the TN Center for the Decision-Making Support virtual resource center and how to use the website resources for future planning.
