Please have your documents ready and complete your application online at APS website at https://www.atlantapublicschools.us/kindergarten. Please refer to the flyer for the required documents.

• Current Lease with Signature or Mortgage Statement
• Current GA Power Bill (within 30 days, Page 1 & 2)
• 3 Supporting Documents
• Parents’ Driver’s License
• Student’s Birth Certificate
• Student’s Social Security Card
• Immunization Record (GA Form 3231)
• Ear, Eye, Dental and Nutrition Records (GA Form 3300)

Affidavit of Residency & Lease Verification will be completed in person at the end of the application process.

The following documents must be UPDATED and CURRENT in your child's permanent record folder. If you have recently updated any of the items below, please submit documentation to Front Office immediately.

• Leasing Contract (With the address and unit number)
• Photo ID (Drivers License or Passport)
• Immunization Form (GA Form 3231)
• Vision, Hearing, Dental & Nutrition (GA Form 3300)

If a student's residence or contact information changes, the parent or guardian is required to notify the school immediately. The following information must be submitted within 14 days of any move or change of address:

• New Leasing Contract with Signatures or Mortgage Statement
• GA Power Bill (current within 30 days, page 1 & 2)

If you are leaving E. Rivers, please complete the ONLINE WITHDRAWAL FORM from HERE.

Please note all APS device and Media Center books MUST BE RETURN TO SCHOOL in order to complete the withdrawal.
