Google Form Submission: Log your individual activities
At least 30 minutes of activity on your own outside of CCTEC (worth 1 point each)
Seasonal yard work/snow shoveling will count if over 1 hour of time. (worth 1 point each)
Participate in fitness classes offered by CCTEC (worth 2 points each)
Participate in special events offered by CCTEC (worth 3 points)
Visit a doctor for a wellness check-up (worth 2 points each)
Qualifying medical visits include: annual physical, annual mammogram/gynecology exam, annual dermatology skin check, annual eye exam, colonoscopy, semi-annual dental cleanings
appointments only count as they are part of preventative care
Staff must earn 22 points from November 1-30th
Remember the Rules for Participation
All points must be submitted by Sunday at 11:59 pm for the previous week.
No points for the month can be added after the last day of the month by 11:59 pm.
Activity points can only be accumulated one time per day. I.e.: a morning workout and afternoon run in the same day is only worth 1 point.

Quarter 1 Award
The end of the first quarter is coming up!
The department with the highest percentage of Wellness Initiative participants at the end of the 1st marking period will receive a healthy lunch to celebrate their overall participation.
Quarter 2 through 4 Awards - Teams have been created!
Give your team a creative name!
An email will be sent to your team wellness participants. Please respond as a group and submit your team name.

In the past we have done things such as healthy(ish) cookie workshops, relaxing craft nights, and staff events on the track.
