5Essentials Returns to Jane Addams
February 1 through March 29 students and teachers in District 15 and across Illinois are asked to participate in the annual statewide Illinois 5Essentials Survey. This survey is designed to generate a detailed picture of the inner workings of your child’s school. As a parent/guardian, this opportunity will allow you to share your thoughts on the important elements of school effectiveness in a survey about your child’s school.
The Illinois 5Essentials Survey gathers data related to five indicators that can be targeted to improve important student outcomes, including attendance and state test score gains. The 5Essentials Survey asks questions about:
- Effective Leaders
- Collaborative Teachers
- Involved Families
- Supportive Environments
- Ambitious Instruction
All District 15 teachers and 4th through 8th grade students will be asked to take this survey. 5Essentials Reports will be generated for schools if at least 50 percent of the teachers and students complete the survey.
We need at least 20 percent of our parents/guardians from your child’s school to complete this survey, a parent/guardian supplement will also be generated for the school and is an important source of information from our families.
Your identity and survey responses are completely confidential. The school does not have access to individual survey responses and cannot connect any survey responses to you or your child.
In District 15, the Illinois 5Essentials Survey for students and teachers will be conducted from February 1 to March 29. You may take the survey between these dates by visiting the 5Essentials Survey website and selecting the appropriate survey to begin.

OPT-OUT INFORMATION
Student participants will be required to log-in using their state student ID and birth date in order to complete the survey. Students’ identities will be completely confidential. Teachers or administrators will never see individual responses to survey questions and will only receive aggregated information, such as the total percentage of students who agree homework assignments help them learn the course material.
Please be aware that under the Protection of Pupil Rights Act. 20 U.S.C. Section 1232(c) (1) (A), you have the right to review a copy of the questions asked of your student(s). Survey questions can be found on the Illinois 5Essentials (https://www.5-essentials.org/).
The student survey window is from February 1 to March 29. If you do not want your child to participate, please fill out this form and have your child return it to their teacher or email it to their teacher by January 31.

January 30 - Half Day (student dismissal is 10:35 am)
February 1 Dual Language Information Night 6:30 - 8:30 pm
February 15 (DATE UPDATED) Full Day Kindergarten Information and Registration Night 6:30 - 7:30 (those families NOT interested in Dual)
February 19 - Presidents' Day Holiday - District closed
March 23 - March 31 - SPRING BREAK
April 1 - School Resumes

Save the Date for the Family Dance! Save the date for the PTO Family Dance on Friday, February 2nd! The Dance will be from 6:00 - 7:30 pm. This is a Family Friendly Event in the JA Gym

New JA construction is home of our new kindergarten wing!
Valentine and Kindness Party Information
We are looking forward to enjoying our Valentine's Day Parties on February 14th. As a reminder, our parties are free of food and treats. If you are sending Valentines or notes of kindness, please consider a small sticker or option other than food.
Our class parties will run from 1:15 - 2:00, if you have any questions, feel free to contact your child's teacher.
Kindergarten Information
Please join us to learn more about Dual Language Programming for potential incoming kindergarten students. An Informational meeting will be held at Jane Addams on February 1 from 6:30-8:30. The first hour of this meeting will be held in English and the second hour in Spanish. If you would like to learn more about our Dual Language program, please plan on attending and watch our Dual Language Informational Video.
Information for our students who are not interested in the Dual Program will be the following week, February 15th (This has been changed from our original day of Feb 8) from 6:30 - 7:30 at Jane Addams. If you can not attend this event but are interested in a tour of the school, please reach out the the school office at 847-963-5000.
