Letter to Parents
Dear Parents & Guardians,
It is with great excitement that we welcome you to Bellmawr School District! We are honored to have you and your family join our school community and look forward to working together to create a memorable school experience for your child. Bellmawr staff are committed to making every aspect of your child’s academic and social development the best it can be. We strive to develop positive relationships with all students to ensure they are safe and supported as life-long learners. We are also committed to providing a rigorous curriculum, utilizing best teaching practices, and actively engaging all students in their academic success so that they can achieve and make meaningful contributions to their respective communities.
We believe that parents and families are an essential element of the learning process, and administrators, faculty, and staff are deliberate about forging meaningful partnerships with them that are centered on high academic and behavioral expectations. One of the best ways to partner together for the benefit of your child is to become an active member of the Parent Teacher Organization (PTO). Registration Requirements
The Bellmawr Public School District has specific registration requirements for students who attend our schools. In addition to meeting residency requirements, the State of New Jersey also requires a physical examination by a doctor and proof of immunizations before entering school. Your child's immunization record, along with the Universal Health Form, completed and signed by your child's physician, must be submitted as part of the registration application. Please note: the immunization record must be attached for the form to be valid.
Please ensure your child's physician completes the form in its entirety. The Universal Health Form includes documentation of medical conditions that might impact your child's health and well-being in the school setting, as well as preventative health screenings that include lead exposure, scoliosis, hearing, and vision.
The district offers in-person appointments for extenuating circumstances where online access is not available to a family. All appointments must be scheduled through our District Registrar by calling Mrs. Barbara Marmarou at 856-931-3620 ext. 1313.
Please fill out all necessary information, save the file, then email as an attachment to
newregistration@bellmawrschools.org.
Attached the completed survey, in the appropriate language, with the registration forms to
newregistration@bellmawrschools.org.

For this form to be valid, a copy of your child's immunization record must be attached.
