Return to School Online Form

If you have already completed the form, you do not need to submit again!

To provide more convenience to parents and eliminate busy paperwork, PLCS Student Return to School Forms are available online. No more completing your name, address and email numerous times on paper. Return to School paperwork must be completed for your child to be enrolled in PLCS.

It is now as easy as 1, 2, 3:

1. Click here to access the form. If you don’t know your Username or Password, you may use the “Need Username” or “Need Password” links to retrieve your information.
2. Complete the form.
3. Sign electronically and submit.

PLCS needs to annually update all student information so we have verified contacts and important safety information for each child. This form must be completed by the parent and submitted, before middle school and high school students can receive their class schedule. For elementary students, the form needs to be completed by the first day of school. The form must be completed for each child by one parent in each household associated with the child.

The username and password is the same one you used last year for the Parent Portal or for the PLCS App. If you do not have that username and password, you can retrieve either or both by following the prompts on this link: Retrieve Username and Password. The retrieved username and/or password will be sent to the email on file with PLCS, the same email where you received this message. If you are new to PLCS and you don’t have an account, you can use this same process to retrieve your username and password.

As a part of the return to school paperwork, parents will be asked to acknowledge the agreement for all middle school and high school students to receive a computer to use at home and school. Parents will also have the option to renew or register for insurance to help cover lost or damaged computers if they so choose. All of this can be completed online. Printing the forms is not necessary.
