SKYWARD HEALTH FORMS
IMPORTANT!!
PARENTAL CONSENT FOR HEALTH CARE SERVICES 2024-2025
A new Florida State Statute (Section 1014.06, Florida Statutes: Parental Consent for Health Care Services) has been released requiring parental consent for Health Services given to your child attending any Orange County Public School. Health Services include a range of items including but not limited to Basic First Aid (band-aids and ice), Hearing/Vison Screenings, and certain therapies. As a Charter School within OCPS, we fall within this requirement.
Please access your Skyward Parent Access and complete the 2024 PCF form for each child attending Orlando Science Charter as soon as possible. At this time any unsubmitted form defaults all answers to NO and Orlando Science is not able to provide students any of the listed Health Services.
Step-by-Step Instructions with pictures:
If you are using a mobile device: After logging in to Parent Access, you will see an icon with three lines at the top right of your page. Upon clicking, use the “Go To Desktop View” link. Once in Desktop View, you will see a plus sign on the left side of your screen. Click that plus sign then click Online Forms. Proceed as instructed above.
If you are unable to access Skyward Parent Access, or are only able to see some of your children’s accounts, please complete the following link so that we may get you access as soon as possible: Skyward Parent Access Issues. Thank you for your patience as we correct any Skyward errors.
