School Medication Pick-up
*End of School Year Medication Pick up– Please pick up ALL medications from the school office by 12pm on June 5th. The school CANNOT store medication in the office over the summer. Any unclaimed medications will be destroyed in accordance with local, state, and federal laws.
*New Health Forms/Meds for the 2024-25 School Year- With each new school year, it is necessary to update your child’s Health Forms for school. All students with medical conditions (i.e. Asthma, Allergies, Seizures, etc.) must submit a new Emergency Action Plan each school year. The forms must be completed (signed by both parent and physician) and turned in to the office when school resumes in August.
*If your child will require medications while at school next year, you must complete a Medication Administration Consent form for each medication to be given at school. Both prescription and over-the-counter medications require a consent form signed by parent AND physician.
*If you would like for your student to self-carry emergency medication, such as an inhaler, epi-pen, or glucagon/diabetes supplies; please be sure to fully complete the "Self- Carry/Administer Medication" section of the Medication Administration Consent form.
*All necessary Medical Forms can be found at the link below:
