Attention Fourth Grade Parents!
Fourth Grade Physicals
Per the Department of Public Health, an updated copy of your child’s most recent physical exam is required to be submitted to the school nurse in Grades K, 4, 7, and 10. Please submit your fourth grade child’s most recent physical exam to Julie Stiles, SRS nurse. You can submit it one of three ways:
Send a copy into school in a sealed envelope labeled “School Nurse.”
Email a copy to me at: jstiles@mpsd.org.
Call your pediatrician’s office and ask the nurse to fax “the most recent physical exam to the school nurse” at: FAX: 339-244-2197.
Any physical dated after June of 2023 will work. Thank you for your cooperation!
Have a great weekend!
