EACH YEAR prior to any practice or participation, a UIL requires a UIL Medical History form signed by both student and a parent or guardian. You all had completed your physicals last year and only need a physical every 2 years.
***FOR INCOMING FRESHMEN 2020***
UIL requires a UIL Medical History form signed by both student and a parent or guardian, and a Pre-Participation Physical Examination (PPE) upon entering the 1st and 3rd years of high school. A physical examination signed by a physician, a physician assistant licensed by a state board of physician assistant examiners, a registered nurse recognized as an advanced practice nurse by the board of nurse examiners, or a doctor of chiropractic is required.
This Medical History form and PPE must be completed prior to July 25 and can be scanned and emailed to Mr. Dixon, or brought to the band registration pick up day on Saturday, July 25.
