Dear Families,

The time is here for students planning to return to District Five for the 2025-2026 school year to complete the annual registration process. Our district has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign all of your registration forms and upload your proof of residency documents for your student(s) earlier than ever before.

Parents are asked to log into FinalForms to verify and/or update their student(s) information. It is very important that this process is completed prior to the start of each school year and is required for all returning students.

The 2025-2026 school year starts on August 7th, 2025.

• If you have forgotten your password, use the “Forgot your password?” button on the login screen.
• Once logged in, your student(s) should be listed on the screen.
• Please do not click “+ Add Student” unless you have a new student to the district.
• If you do not see your student(s) listed, please contact the school registrar.
• Complete the forms for each of your students by signing and submitting them.
• Use the following link to sign into FinalForms: https://lexingtonrichland-sc.finalforms.com/.

Proof of residency is required yearly for all students in School District Five of Lexington and Richland Counties. Proof of residency documents from the 24-25 school year may not be used to complete registration for 25-26.

Parents: If your student has to attend summer school you will be contacted via email with registration information and additional details.

(There is NO enrichment offered at summer school.)

• Week 1-June 9-12
• Week 2-June 16-19
• Week 3-June 23-24

One Location: Irmo Middle School from 08:30AM to 01:30PM

Parents: Drop off your students to DFMS no earlier than 07:50

*Transportation will be provided from DFMS to/from IMS: Bus leaves at 08:05am

* Transportation from IMS to DFMS: 02:00pm

Parents: Pick up students from DFMS no later than 02:15 pm

Parents: You will be contacted through email with registration information.

If your student will not be returning to Dutch Fork Middle School or Lexington/Richland School District Five for the 2025-2026 school year, please contact the Office of School Counseling to begin the withdrawal process. The student's district issued Chromebook, case, and charger will need to be returned to the school, as well as any library books that are checked out. Any outstanding fees will need to be paid as well. If you have any questions, please contact the Office of School Counseling at 8003-476-4805.
