Each player will need to fill out and sign the Teen Center’s Registration/Waiver form. They will be collecting them at the Teen Center front desk. The .pdf is attached below.

Please select the Council(s)/Committee(s) that you would like to receive information and updates on, approximately monthly. If you have students at multiple school sites, please submit the form for each school. Additionally, if you opted-in last year, please plan to do so annually. Importantly, should you choose to opt-in to receiving these updates, the parent/guardian representative for the Advisory Council(s)/Committees(s) and/or the administration will not share your email address with others. You also have the ability to opt-out at any time in the event you no longer wish to receive the Advisory Council/Committee information.
