Fall Sports Begin August 13, 2025
**IMPORTANT – PLEASE READ ALL INSTRUCTIONS
NEW Forms for Sports Physicals Beginning SY 2025-2026
Registering for high school sports involves:
- completing the online athletic registration at Form RELeaf
- turning in a sports physical.
Form RELeaf is required for each sports season. Only one Sports Physical is required each school year.
Student Athlete and Parent/Guardian Check list for Sports Registration
_____ 1. Please make sure to read all the information that your school provides about Eligibility, Expectations, Tryouts, Practice & Game Schedules, Transportation (to and from games), Login to the School System Registration website (FormRELeaf).
_____ 2. Page 2: Health History form. This is filled out by the student athlete & parent/guardian. Please fill out the Student Athlete Heath History form, take it to the Pre-participation Physical Exam (PPE) appointment and review with the Healthcare Professional. Make sure to clarify/explain any questions that you have answered “YES”. Please keep a copy to turn into the school.
_____3. Page 3: Pre-participation Physical Exam (PPE). This will be completed by a Medical Doctor (MD),
Doctor of Osteopathic Medicine (DO), Certified Registered Nurse Practitioner (CRNP) or Physician Assistant – Certified (PA-C) only. Sports exam date must be on or after June 1st of the school year of intended participation.
Pre-participation Physical may not be completed/signed by a parent/guardian even if they are a licensed healthcare professional.
• Before leaving the appointment, please make sure the following have been completed:
o ___ The Healthcare provider signed, dated, and stamped the PPE.
o ___ The Healthcare provider has checked off the appropriate participation in athletics box.
o ___ You have both the Health History form and Pre-participation,
Physical Exam (PPE) form. (you will need to provide both forms to the school during sports registration)
_____ 4. Page 4: Emergency Information Form (to be completed and signed by parent/guardian). This information will be shared with the coach(es) in case of an emergency at practice/game.
_____ 5. Students who require medication at school (including during school team practices or games) must have a doctor's order on file with the school's nurse for each medicine. Please visit this link and take this form to your Healthcare provider for school medication administration authorization. Self Carry Medication Order Form (This needs to be completed each year)
Students are not allowed to self carry any other medications in their sports bags.
The information provided on the Health History and Pre-Participation Physical is considered confidential medical records, it is established and maintained for every student. The confidentiality of a student's medical records information is protected under the federal Family Education Rights and Privacy Act
(FERPA), Maryland state law and/or the local school system policy, as applicable.
The pre-participation physical examination is not a substitute for a thorough annual examination by a student’s primary care physician. Sports exam date must be on or after June 1st of the school year of intended participation.
WHERE TO TURN FORM IN
- There will be a desk with a YELLOW Box on it outside of Health Suite 1 (Main Hall) as each sports season approaches. SUMMER TURN IN--the YELLOW BOX may be in the Main Office for some part of the summer.
- Please turn in all 4 pages of the sports physical packet
- School Nurse will be processing the forms as they come in.
- Email of form is not preferred. If you email, we still need original for clearance.
Where Can Students Get A Sports Physical?
Sports Physicals are different than Annual Medical Physicals done at the Pediatrician’s Office.
A sports physical focuses on reviewing your child's current health status and medical history to ensure that your child is healthy enough to play his or her sport.
Sports Physicals are usually not covered by insurance.
Note: All exams must be dated on or after June 1, 2025.
There are several options to where a Sports Physical can be done:
NOTE: TAKE HCPS Sports Physical Packet WITH YOU TO PROVIDER DOING THE SPORTS PHYSICAL!
1. Urgent Care Centers–Sports Physicals are most often done at Urgent Care Centers.
The exam and paperwork is done at same time. Cost is $15 and up (check with the center you are going to when you schedule or walk in). Take the Sports Physical Forms Packet with you!
2. CVS/Target Minute Clinic– Sports Physicals can be done at CVS/Target Minute Clinic. The exam and paperwork is done at same time. Cost is $15 and up (check with the location you are going to when you schedule or walk in). Take the Sports Physical Packet with you!
3. Pediatrician/Primary Care Provider– Sports Physicals can be done at your student’s primary care provider/pediatrician’s office. If your provider is filling out the forms (provide them with the Sports Physical Packet) it is VERY IMPORTANT that that exam date and date on form is ON OR AFTER JUNE 1, 2025. (Many providers will fill out forms, but the form is based from an exam*prior to filling out the forms. *Exam date MUST BE on or after June 1, 2025. Costs for Sports Physicals by pediatrician/primary care provider varies by provider
