New Student Enrollment Information

NOTE: If you previously registered for Pre-K or Kindergarten in the Spring, DO NOT re-enroll!

Please take a moment to review the required enrollment documents below. You will be asked to upload them at the end of the form. If you are unable to upload your documents, please provide any missing documentation to:

• Pre-K: Jennifer Dieter, Coordinator of Pre-K Programs, jdieter@k12.wv.us.

• Grades K-8: Summer Registrars, summer_enrollment@bcswv.org
• High School: please contact your student's school.

BCS utilizes JotForm, an online platform, to complete new student enrollments. In order to successfully enroll, all parents/caregivers will need to provide the following documents for their students:

• Student's State-Issued Birth Certificate
• Birth Certificate Affidavit (if applicable)
• Proof of Residency
• Student Immunization Records
• Most Recent Health Check/Physical
• Court Documents/Custody Papers (if applicable)
• Administration of Medication Form (if applicable)
• Medical Plan for School Food Services (Special Diet) Form (if applicable)
• IEP/504 Plan (if applicable)

Requested Documents:

• Social Security Card
• Last Report Card (if applicable)

Parents and caregivers interested in registering their student(s) for the Berkeley County Virtual School pathway are required to take a Readiness Quiz to determine if the program is the appropriate fit. Completion of the survey does not guarantee placement into the BCVS pathway, and the results will be sent to the appropriate school leadership for more information.
