If you have not done so already, please complete the form by clicking the button below. This must be completed for each child in your household. This form is the internet use agreement, photo consent, and permission for walking field trip.

Important paperwork came home with students last week. A few things to note:

--Trip accounts must be PAID IN FULL by October 21
--There are two forms that need to be completed:

1. Eagle Bluff's Medical/Liability Form--click HERE (online only)
2. Sibley East's Overnight Field Trip Health Form--(paper copy only)

Note: We will have a "Chaperones" meeting in early October. Until then, please reach out to Mrs. Feterl (amanda.feterl@sibleyeast.org) with questions.
