An Explanation of Benefits (EOB) is a statement from your insurance company that outlines the costs associated with your doctor or clinic visits. It serves as a notification that a claim has been filed and provides a breakdown of the expenses.

Unlike a bill, an EOB simply informs you of what your insurance has covered and what portion you are responsible for paying. Your healthcare provider will send a separate bill for your share, which should align with the details on your EOB.

• 100% Covered Preventive Care – Exams, cleanings, X-rays, and more (no deductible!).
• Basic Services – Fillings, root canals, and extractions covered at 80%.
• Major Services – Crowns, bridges, and dentures covered at 50%.
• Orthodontics – Braces for children up to age 18, covered at 50% (lifetime max: $1,000).
