Looking to participate in a winter sport? Follow these easy steps!
IF YOU ARE A RETURNING ATHLETE
Please note: ALL ATHLETES NEED TO UPDATE/COMPLETE A NEW MEDICAL HISTORY FORM ON SPORTSWARE AT THE START OF EVERY SPORTS SEASON. You will not be able to participate on the first day of tryouts until this is done.
2. Please be sure to declare your desired winter sport under “Sport 2”.
3. Be sure that your physical is up to date. Sports physicals remain valid for one (1) year. If you received a sports physical when the mass physicals were offered during school in spring of 2024, your physical is still valid. If you are unsure of your physical status, please contact Mrs. Gailor by email at cgailor@lockportschools.net Alycia Rich at ameek@lockportschools.net or Rosie Heinemann at rheinemann@lockportschools.net and they can check for you.

Once you set up the account, you will be prompted to complete the three required documents:
-Parent Student Concussion Form
-Parent Permission Form
-Medical History
Once you have completed the documents, you will need to print out the completed Medical History form and have your son/daughter bring it with him to the physical appointment. If you are unable to print at home, please email Mrs. Gailor at cgailor@lockportschools.net by no later than Friday 11/15 at 12:00pm with your child’s name to request the form be printed for you and given directly to the athletic trainer.
We realize this process may not come easily to everyone. We are here to help! If you require assistance setting up your account, please contact Mrs. Gailor to set up an in-person appointment to help you navigate your SWOL account.
- Helpful Hint: If you are a parent of multiple student athletes- you will need separate SportsWare accounts for each child. You are able to use the same parent email for multiple accounts, but are not able to have the same email/password combination for any two accounts. If you are using the same email for multiple accounts, please be sure to use different passwords for each account.
2. Set up an appointment for a Sports Physical.
UBMD Sports Physicals will be offered at Lockport High School, free of charge, on Friday, November 15, 2024 by appointment only starting at 2:30pm.

Please be sure to bring your printed and completed Medical History form from your SportsWare account. Again, if you are unable to print at home, please email Mrs. Gailor at cgailor@lockportschools.net by no later than Friday 11/15 at 12:00pm with your child’s name to request the form be printed and given directly to the athletic trainer.
If you miss the physicals or are unable to make it on November 15th, you will need to make an appointment at a UBMD Sports Medicine office by calling 716-204-3200. The appointment will cost you $40 cash and you will need to bring your printed Medical History form from your child’s SportsWare account with you, as well as a NYS Health Exam form.
If you are unsure of your child’s current Sports Physical status, please feel free to contact Mrs. Gailor and she will check for you.
IF YOU ARE A 7TH OR 8TH GRADER LOOKING TO TRY OUT FOR A JV/VARSITY SPORT
We are proud to say that we offer a Modified Sports Program for 7th & 8th grade students. Modified is the proper sports level for this age group. If your son or daughter wishes to try out for a sport at the high school level (Varsity or Junior Varsity), they would need to pass all aspects of the APP Test. Please note: Bowling does not require passing of the APP Test. The APP test consists of the following:
1. Pass the Tanner (Physical Maturity) Test. Our district physician conducts these tests for our athletes at the APP Test. Parents are welcome to be present for this part of the exam, but please note that no parents are allowed in the gym where the APP test is taking place. If you would prefer to have your child’s primary care physician conduct this exam instead, you will need to make an appointment at your child’s doctor’s office and have them complete the Physical Maturity Form.
2. Pass four of the five components:
Curl-Ups
Shuttle Run
Pull-Ups OR Right Angle Push-Ups
V-Sit Reach OR Sit & Reach
1 Mile Walk/Run OR Yard Swim
Please note: Passing scores are relative to the child’s age at the time the test is taken.

3. Once your child passes the APP test, they are cleared to tryout for the upper level sport of their choosing.
Please note: Passing the APP test does not guarantee your child a spot on an upper level sports team, it merely allows them to participate in tryouts.
4. If your child passes the APP test but does not make an upper level team, they are always able to then tryout for the modified level sport of their choosing.
The APP test will be offered at Lockport High School on Friday, November 15th at 4:00pm by appointment only. Please contact Mrs. Gailor by email at cgailor@lockportschools.net or by phone at 716-478-4474 to set up an appointment. Please enter through the Locust St Bus Loop entrance (Door 3), and please send your child with the required Parent Permission Form.
**Student-athletes can take the Sports Transfer bus from AMJHS to LHS immediately after school lets out on 11/15 (Bus #703 in Slot 1)- just be sure to send them with their signed Parent Permission Form, or they will not be able to take the test.
