This means that children who are not up to date on their immunizations by this date will not be allowed to attend school.

What does this mean?

• Children must have all required immunizations or a medical or non-medical exemption to attend school

• Children who are not up to date will be excluded from school until they are immunized or have an exemption
• Parents can contact their child's medical provider or local health department for help

Why is this important?

• Immunizations are one of the most effective ways to prevent the spread of serious diseases like measles, mumps, rubella, and whooping cough

What can parents do?

• Review the immunization requirements for their child
• Fill out a Certificate of Immunization Status Form with their child's immunization dates
• Sign and submit the form to their child's school
• Contact their child's medical provider or local health department if they receive an exclusion notice
