The physical and updated vaccines are mandated for entry into school for children entering the following grade levels:
- All new and early childhood students
- All kindergarten students
- All sixth-grade students
These forms are due before the start of the school year, no exceptions. Please check with your child's pediatrician should you have any questions regarding your child's vaccine status.
Dental forms are required for the following grade levels:
- All kindergarten students
- All second-grade students
- All sixth-grade students
These District 103 students are required to present appropriate proof that they received a
health examination and the immunizations against, and screenings for, preventable communicable diseases. Review the immunization requirements for the upcoming school year to ensure that your student has received what is required in order to start school on the first day of class. Please visit the D103 Health and Wellness website for specifics on immunizations, allergies and food guidelines, and medications and management plans. Please contact your child's school if you have additional questions. If possible, please return forms to your child's school nurse by August 1.
If you are planning on playing a sport at Daniel Wright this year, a sports physical will need to be on file with the school nurse. Sports physical and medical forms are valid for 13 months.
