Incoming 6th Graders for 2024-2025 - Medical Checklist
With summer fast approaching, we are beginning to plan for the upcoming school year. Please see the attached RBMS Medical CHECKLIST which reviews all required medical paperwork due BEFORE the first day of school. Per District 47 policy, 6th graders must have a school physical and updated vaccine record prior to the first day of school. Unfortunately, should those forms not be submitted prior to the first day, your child will be excluded. Should your child have a medical condition requiring medication to be given at school and/or classroom accommodations, please have your physician complete the attached medical forms on the RBMS Medical Checklist. Should you have any questions or concerns please don't hesitate to contact me.
Thank you and have a great day!

Are you Moving?
Are you planning on moving over the summer and will not be attending D47 in the Fall? If yes, please contact Mrs. Smith Administrative Assistant at the RBMS school office by May 24th.. RBMS will transfer your student's records to the District 47 Office before the summer. When you register at the new school, have the new school reach out to the District 47 Office, not RBMS, for the students records.
