*HSE HIGH SCHOOL IS HOSTING THE MOBILE DENTISTS ON 10/7/2024
HSE Schools welcome the Smiles Program/Mobile Dentists to provide a free exam and cleaning to your student while they are in school. We understand many families do not have dental insurance and cannot afford the costs out of pocket, so we hope this will provide the dental care your student needs. Mobile Dentists accepts dental insurance as well, so please provide that information if you’d like to register your student(s).
To register your student(s), please complete a form by using one of the following methods:
- Go to: www.myschooldentist.com, find your school in the listing and complete the online form
- Scan the QR code below to take you to the website
- Copies are also available at the school front desk. You are welcome to stop by or your student can pick up a copy from your building nurse
Please complete all information on the form. If information is missing, you may be asked to complete another form. You may submit the form by 10/4/2024 to ensure Mobile Dentists have time to process the forms prior to the appointment date. If you have any further questions, feel free to go to the website: www.myschooldentist.com or contact Amy Hanna, Director of Health Services ahanna@hse.k12.in.us.

Where: HSE High School
Price: Students need to purchase a ticket on Gofan starting Friday 9/6 until the day of the dance. Cost is $15 per ticket until Friday 9/27 at midnight. What to Wear: Semi-formal attire. Girls usually wear a short dress or pants outfit. Boys usually wear dress pants with a button-down shirt and tie. Suit jackets can be worn but many boys do not. Dress shoes are not required so kids can wear any variety of shoe they feel comfortable in.
Drop Off and Pick Up: Please drop your student at door 18 and pick up at door 18 on the day of the dance. The dance ends promptly at 10:30pm so please plan to get them as close to that time as possible.
