• A completed well-child physical exam is a requirement for students entering Kindergarten and 6th grade students. If you have not already, please submit a completed private physical exam. A physical exam from 8/1/2023 or after will meet the requirement.

• Completed paperwork may be faxed to Larry J. Macaluso Elementary: (223) 200-3171 or email the well-child summary (AVS - after visit summary) from the MyWellSpan portal if your child participates with a WellSpan provider to sminkc@rlasd.net (see instructions below).

• If your child's physician is requesting a physical form to be filled out, please see the form here.

• Dental form reminders:

• The state of Pennsylvania requires a dental exam upon entering Kindergarten and 3rd grade. Any dental visit from 8/19/2023 (One year prior to the start of the current school year) or after will meet the requirement. The form can be found here.

If your child does not have a dentist, our certified school dental hygienist can provide exams. The form can be found here.
