- 3rd grade will enter through Door C
- 4th grade will enter through Door F
- 5th Grade will enter through Door E
See you soon!
Mrs. Peacock
***Important Reminders from the School Nurse***
- If your student will require any medication, (prescribed or over-the-counter), during school hours, please have the required District 153 Medication Authorization Form filled out by their physician and return it to the office.
- The Medication Authorization form can be found on our district website in the Health Services section along with Emergency Care Plans, or you can get a copy in the Churchill office between the hours of 8am-3:30pm Monday-Friday.
- New/updated Emergency Care Plans and Medication Authorization forms are required each new school year for their safety.
- Students are not allowed to self-administer medication without a physician’s letter.
- Please do NOT send in any medication without these forms.
- Medications must be in the original bottle/box received from pharmacy.
- No need for any new school physicals, immunizations, dental or eye exams for 3rd-5th graders unless their immunizations are NOT up to date or if you have a NEW STUDENT.
- Religious Exemptions are required with each new school year.
- This year for lunch we will substitute Lactose Free Milk or Soy Milk, in place of cow's milk, only to students that have a Physician's Note on file with the school or they can purchase it through their lunch account. This is to remain in compliance with the National School Breakfast and Lunch Programs.
- Any student with a fever of 100.4 or greater must be fever free (without the use of fever-reducing medication) for 24 hours before returning to school to prevent the spread of any contagious illnesses.
Please send in those very important forms as soon as possible, preferably before the start of school. You can email Nurse Marcano with any other important information such as the need for “Nut Free Table/Classroom”, etc. You can also meet her on Supply Drop-Off Night.
