The time is here for students planning to return to District Five for the 2025-2026 school year to complete the annual registration process. Our district has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign all of your registration forms and upload your proof of residency documents for your student(s) earlier than ever before.

Parents are asked to log into FinalForms to verify and/or update their student(s) information. It is very important that this process is completed prior to the start of each school year and is required for all returning students. The 2025-2026 school year starts on August 7th, 2025.

• If you have forgotten your password, use the “Forgot your password?” button on the login screen.
• Once logged in, your student(s) should be listed on the screen.
• Please do not click “+ Add Student” unless you have a new student to the district.
• If you do not see your student(s) listed, please contact the school registrar.
• Be sure to click the “CLICK TO SWITCH TO 2025-26 REGISTRATION” button found above the list of your student(s).
• Complete the forms for each of your students by signing and submitting them.
• Use the following link to sign into FinalForms: https://lexingtonrichland-sc.finalforms.com/.

Proof of residency is required yearly for all students in School District Five of Lexington and Richland Counties. Proof of residency documents from the 24-25 school year may not be used to complete registration for 25-26.

Please read all of the information in this letter carefully to understand School District Five’s end of year procedures for district devices.

1. Students in grades 3-5 returning to any D5 school for 2025-26 will keep their district issued devices over the summer. If a device is damaged during the summer, students should contact their current school. Please make sure you take good care of your device over the summer and return in August with the device in good working order.

2. All current K-2 students will turn in their devices, cases and chargers to the school before the end of the school year and will not have district issued devices over the summer. They will be issued devices at the beginning of the new school year. All devices and charges will be turned in on Tuesday May 27th.

Per the District Issued Device Guide, the district covers one (1) accidental damage incident during the 2024-2025 school year (August 1, 2024 - July 31, 2025). In order to be considered for 2024-2025 coverage, incidents should be reported by July 31, 2025. Any accidental damage reported after this date will be classified as part of the 2025-2026 school term.

If the device currently has any issues or damages, please do not wait until the end of the year or collection time to notify the school’s help desk.

• Charger - up to $20 (missing, damaged or incorrect charger)

• Damaged devices will be reviewed by the Technology Services Department. Parents will be notified if charges are necessary.

• Costs are assessed based on replacement price which is dependent on age and model of device and charger
