Illinois 5Essentials Survey - LAST WEEK!
Oak Lawn-Hometown School District 123 is conducting the 12th annual Illinois 5Essentials Survey, in collaboration with the Illinois State Board of Education, to evaluate school climate and learning conditions. All D123 schools must have a minimum amount of participation in the survey to receive results.
The survey, open from January 30 to March 29, invites students (grades 4-8), teachers, and parents to provide feedback aimed at enhancing educational experiences and the school community. Focusing on five critical aspects—Effective Leaders, Collaborative Teachers, Involved Families, Supportive Environment, and Ambitious Instruction—the survey results will contribute to the 2024 School and Illinois State Report Card, ensuring anonymity for all participants. Those wishing their student not to participate must submit an opt-out form to their child's school office by February 9, 2024.

Introducing the D123 4-Ward Alumni Program
We want to hear about our alumni's successes in high school, college, and beyond! As our students graduate from D123 and move 4-Ward, we want to stay connected and hear about all the journeys taken in "tomorrow's world." Tell us stories from the classroom, the athletic field, the stage, the culinary lab, extracurricular activities, volunteer experiences, and everywhere in between. We are interested in everything our alumni have been up to since they graduated 8th grade. Fill out this form to submit for our alumni spotlight electronic newsletter, D123 4-WARD.
