Updates from the Health Office
Welcome Back!
As we begin our new school year, I wanted to take a moment to send some reminders from the Health Office.
Health Forms
Thank you to all families who have already submitted Health Forms. Students entering Preschool, PreK 4, Kindergarten, and Sixth Grade have required vaccinations, as well as Universal Health Record (PS, PK, K) and Preparticipation Physical Evaluation Medical Eligibility Form (6) this school year. Please review the Health Requirements document under your child’s specific grade level on our website. You may access this by going to the Parents tab at the top of our Home Page, then clicking Health Forms. For students with specific medical needs like allergies or asthma, you will also need to complete the forms for those needs. Forms should be sent to school prior to and through the first week of school. Please include vaccination records, which can be obtained from your child’s physician. All forms must be submitted by September 30, 2024.
For students in grades 1-5 a Universal Health Record is to be submitted yearly, completed by your child’s physician, following your child’s yearly physical.
For students in grades 7 & 8, the Preparticipation Physical Evaluation Medical Eligibility Form is to be submitted yearly, completed by your child’s physician, following your child’s yearly physical.
If your family has chosen to exercise Religious Exemption for vaccinations, I will need a letter stating this, signed by both parents, and notarized. If you have questions regarding this, please feel free to contact me individually.
Medications
The school office will be available to receive inhalers, epi pens, and other student medication beginning Tuesday morning. Please note that all student medications should be in their original containers, complete with prescription label or in an unopened original box labeled with student name and date of birth for over the counter medications, and must be accompanied by the Medication Form, found on our website, signed by parent and physician.

Attention Needed - Complete the Emergency Contact Form
All families must complete the Emergency Contact Form linked below. Please complete and return with your student on Tuesday, September 3rd. In addition, if there has been a change in any custodial agreements or in your permission to publish form, please reach out to Mrs. Bakas at abakas@allsaintspar.org
Dismissal Procedures - Car Line

View the video clip above for K-8 dismissal. Siblings of students in PS and PK will dismiss with the younger siblings - procedures to be discussed at orientation. After Care is organized in the cafeteria. Parents may drive to the rear of the building and proceed to the cafeteria doors for pick up.
