REMINDER - Please return forms

Students will be given a packet the first day of school which included required forms to be filled out and returned. Students may return the completed forms to their teacher in crew class or drop them off in the main office.

● Family Contact Information – This information needs to be updated each school year. Please complete both sides of this form and return it to the main office or crew teacher.

● Free & Reduced Lunch Form – This form MUST be returned whether your family may or may not qualify for free or reduced lunch. Lunch Forms - English & Spanish

⮚ Should you feel your family will or may qualify free or reduced meals, please complete the entire attached application, sign/date and return to the main office or crew teacher.

⮚ Should you feel your family does not qualify for free or reduced meals, please list child’s name on step 1, write “waive process” on Step 3, and sign/date Step 4 of application and return to the main office or crew teacher.

Your assistance is appreciated in returning the required forms and wishing you and your child a school year full of success.

Ocean State Transportation - SafeStop app

Our school bus provider, Ocean State Transportation, has started using the SafeStop app for the 2024/2025 school year, which allows families to track school buses in real time. Please see the flyer below and visit https://safestopapp.com/ for instructions on how to download and use the app.

She will also cover essential topics like health risks, legal consequences, and peer pressure when it comes to traffic safety.
