Transition is not a once a year or yearly process, it is ongoing. So, this is a check in/reminder to monitor the progress of all students transition plan and progress. Where are we in monitoring our students' progress toward their postsecondary transition goals/activities/services? Have we been monitoring our students' transition activities? If you haven’t already, which I am sure you have, but in case you have been overwhelmed, it may have been missed. Remember transition plans can be updated during the year to reflect changes in students' postsecondary goals.
Transition Progress Monitoring
*Is your data collection for transition activities/services for each student on your caseload up to date?
*Is every student on track to complete their postsecondary activities by proposed dates via their transition plan IEP date?
*Have students received the postsecondary services via their transition services stated/listed on their transition plans?
*Have the activities been achieved, if so, should the transition activities in the IEP be amended or reconvened to add more activities to assist students in attaining their postsecondary goal(s)?
Are transition assessments updated, accurate, or do they need to be reassessed?
Some additional things to look at:
Are you documenting supplemental aids and services?
Are the Medicaid services information for students updated and put into PSSP by the 15th for the previous month?
Do any discipline records need to be reviewed, or days counted?
Are the accommodation(s) prepared/updated for the upcoming local/state assessments?

Washtenaw County Post-Secondary Transition Manual
This Tuesday’s Transition Tip is a review to some and introduction to others. It is important as special education providers/case managers that we create structured and detailed postsecondary transition plans for our students. The Washtenaw County Post-Secondary Transition Manual is a guide to assist special education staff with creating complaint post-secondary transition plans. This guide contains a table of contents to help locate information quickly. The manual also gives insight on how not only to build a complaint transition plan but a quality transition plan. It includes links, videos, and an appendix to assist. Please take the time to read and review the Washtenaw County Post-Secondary Transition Manual.
