As youth transition from pediatric to adult healthcare services, it's important to understand the differences they will encounter. This presentation will help youth become familiar with the process of engaging with adult providers and how the adult healthcare system works. This presentation will also help youth learn how to prepare for an appointment, what to take to the appointment, and what to expect at the appointment.

Family S.T.A.R. supports families with children and youth from birth to age 26 throughout the state who have special healthcare needs, complex medical conditions, developmental disabilities, and mental health challenges, along with the professionals, providers, and educators who serve them. Our services include Support, Training and Education, Assistance, and Resources and are offered at no charge. Family S.T.A.R. is Florida’s Family to Family Health Information Center (F2FHIC), funded by the U.S. Dept. of Health, under the Health Resources Services Administration.
