Parental Consent for Health Care Form

The Health Office is staffed by an RN and backed up by trained office staff. Please note, per
Idaho Code 32-1015, if consent to treat is not given, your child will not be treated for any injury
or illness unless your child has a life-threatening emergency event. Emergency services may be
rendered by the Boise School District (examples: CPR, Epinephrine for anaphylaxis, or Narcan
for possible opioid overdose). I Thereby CONSENT to the treatment of our minor child by a
Boise School District designated staff member for same day illness or injury to our minor child
while on or adjacent to any school grounds of the Boise School District. This includes Boise
School District sanctioned activities, trips, or events. Illnesses or Injuries, non-school related,
that occur after school hours will be referred to a community provider for treatment. This consent
shall terminate as soon as any of the undersigned are contacted, in which case further medical
treatment can be done only with the consent of the person contacted. This consent shall be
valid unless and until revoked in writing by one of the undersigned. This consent does not
extend to the administration of vaccines, immunizations, or treatment as a result of sexual
activities (STD's, pregnancy etc). Parents will be provided referrals for these circumstances
unless the individual is 18 or emancipated.

Does your student need extra support?
Borah is offering support groups to all students and are accessed by both those seeking connections and those needing intensive support. The purpose of a support group within the high school setting is to provide participants with a comforting, safe environment where they feel free to express themselves. In addition to providing emotional support, group participation also allows students to establish meaningful and trusting relationships with their peers. The Spirit Squad has two fundraisers that are coming up and we would love for you to help us out! Mark your calendars for September 13th from 11am-6pm. We are hosting the Ford Drive 4 Your School Event and need people over the age of 18 to test drive a car at Borah that day. If you stop by during that time frame the Spirit Squad will help you fill out the waiver form and pick a car to test drive. We would love your support.

Library News
The Borah Library is open every day 7:30-3:20. Students can enjoy after school study time in the Library when “Lions Den” kicks off after Labor Day. Lions' Den runs Tuesdays-Thursdays 3:20-5pm. We assist students daily in finding the perfect book, be it print and/or eBook/Audiobook. Students complete, print and submit assignments here. Students can access a plethora of professional and academic news and research through the “Research & Databases” page on our website. This is a 21st century library commons and we encourage students and classes to make use of our diverse collection of technology and tools: audio/video recorders, editing software, iMac, Adobe Suite, 3D printer, laser & vinyl cutting tools, craft supplies and games. See all of this on our library website We’re here for you 24/7! Also, if your student has any issue with their BSD issued Chromebook the Library is here to help!
AP Testing Updates, Deadlines, and Fees
All AP exams this year will be in some digital format with the exception of Spanish Language, French, and Music Theory. Students will use their school issued Chromebook to take their exam in May.
The deadline to register for AP exams is November 15th.
The deadline to drop an AP class this semester is October 11th.
The College Board fee to drop an AP exam after November 15th is $50.
