We have sent home some forms that need to be filled out and returned by Friday, March 28, 2025. Please make sure to check your scholars backpack for these important forms.

• Student Housing Questionnaire. This form is asking if you own or rent your apartment or home. If you do, simply initial on the line next to the highlighted text. If you do not, please check all the boxes that apply and fill out the bottom portion of this form. This is to make sure you are receiving any services you may be entitled to under the McKinney Vento Act 42 U.S.C. 11435. This form is due on Friday, March 28, 2025.

• Student Information Update Form. This form allows you add or make any changes to phone, address, email address or emergency contacts. This form is due by Friday, March 28, 2025.

• Big Smiles Dental forms. Big Smiles Dental will be at Cascade View April 25, 28 and 29, 2025. If you would like your child to be seen, please return the form by Friday, March 28, 2025.
