Changes to 1095-B Forms for Employees
As part of the Tax Cuts and Jobs Act, the mandate that required individuals to have minimum health coverage or pay a tax penalty has been permanently removed. Due to this change, the IRS no longer requires insurance carriers to mail out 1095-B forms.
How to Access Your 1095-B Form:
Employees enrolled in a Premera health plan can access their 1095-B form through one of the following three methods. The updated forms will be available before March 1, 2025.
- Sign in to your Premera account and go to My Account/Tax Form 1095 to download and print the form. Or go to Secure Inbox if you’d like to email your request to have the tax form mailed.
- Call the customer service phone number on the back of your ID card to request the form be mailed.
- Complete the 1095-B Mailing Request Form and mail it to the address listed on the form.
Why You Need Your 1095-B Form:
The 1095-B form provides proof of the health coverage you had during the year, which may still be important for your personal tax filing.
