Kindergarten Registration
Kindergarten Registration for 2025-26 is now open!
In order for your child’s registration to be complete, he/she must be 5 years old on or before September 1, 2024, and the following information must be completed:
□ School registration form/or on-line registration for those new to HSD https://www.hsd.k12.or.us/Page/6083
□ Immunization form
□ Home Language Survey
□ Digital Resources Permission Form
□ Dental Screening Certification
□ Proof of age/name (any one from the following list)
· (preferred) Original or certified copy of birth certificate
· Current passport or Alien registration card.
· Immigration or naturalization documents
· An adoption decree or adoption certificate issued by a court of record
· A Certificado de Matricula Consular (Mexican Consulate ID card)
· hospital or baptismal record
· Proofs of identity include court papers or court orders, such as legal name changes, issued by court of record.
□ Proof of Address
Oregon state law requires all students 7 and younger to have documentation of vision screening within the first 120 days of school. HSD will provide that screening for all kinders; if you have had your child screened previously, check with the office for a certification form.
Our office will be closed for summer break beginning June 24. In order for us to plan for the number of students who will make up our classes, we need your registration as soon as possible and hopefully by June 10. The office will reopen mid- August.
If you know of other families with children of kindergarten age who need to register, please encourage them to call or come into our school.
Birthday Treats
We kindly request that parents do not send treats or snacks to school for birthday celebrations. This promotes inclusivity, healthier choices, and focused learning. Some students have dietary restrictions or allergies that prevent them from enjoying certain treats. By not having treats, we can ensure that valuable class time is not disrupted and that our students can fully engage in their lessons.

Ladd Acres is a Fragrance Free School
Please be courteous and not wear or send your students wearing colognes, perfumes or other scents. Colognes/Perfumes/scents, can cause sniffling, dizziness, headaches, nausea and breathing problems for many students/staff members.. Many staff and students have health conditions (asthma, migraines ) that can be triggered by fragrances. NOT using fragrances at Ladd creates a healthier environment for everyone. Thank you!
