Massachusetts Immunization Requirements for Secondary Schools
Students are only allowed to attend school if they have the proper immunizations. The Massachusetts Immunization Requirements for Secondary Schools Students states:

Please review the regulations pertaining to school immunization requirements for further information: 105 CMR 220.00: Immunization of students before admission to school
For High School students, please carefully review the updated Meningococcal Requirements:
Grade 7-10

Annual Physical Examinations: Physicals are mandated by Massachusetts for students entering grades 1, 4, 7, and 10 (and for any new or transferring student to the school district). The physical examination must be on file at the Health Office within 2 weeks of starting at that school.
Athletics: All students participating in an interscholastic sport must have a physical every 13 months. A copy of this physical must be on file in the Health office before the first practice (parents/guardians of students at the High School must upload the physical through their FamilyID account: https://students.arbitersports.com/programs/holliston-h-s-fall-athletics-registration-2024). Please be aware that FamilyID has recently been acquired by Arbiter Sports.
Medications: All medication orders and parent/guardian consent to administer the medication need to be renewed at the beginning of the academic year and/or as necessary during the academic year. Instructions on how to drop off medication for your student and/or addition forms can be found on our website: Holliston Public Schools Health Office. We can only permit students to carry 4 types of medications (EpiPens, inhalers, enzyme supplements, and insulin delivery system) and the school nurse must provide written consent.
All medication that is to be stored in the Health Office must be delivered by a parent or guardian to a school nurse. Health Office nurses will be at each school on the following dates for medication drop off prior to the first day of school for the 2024-2025 academic year:
Placentino School Health Office: Tuesday, August 20th; Wednesday, August 21st (8:30 am - 2 pm both days).

Please ensure you have all the completed documents detailed in the Medication Authorization Packet when you drop off the medication.
