***THE DEADLINE FOR ALL SPORTS PAPERWORK IS

ALWAYS 2 WEEKS BEFORE THE SEASON START DATE

ALL PPE MEDICAL ELIGIBILITY FORMS AND GENESIS FORMS FOR

WINTER SPORTS & INTRAMURALS

MUST BE SUBMITTED BY

OCTOBER 29TH

It is the responsibility of the parent/guardian to complete and submit all Sports PPE Medical Eligibility Forms and Genesis Sports Forms before the deadline.

If you cannot meet the deadline, please inform your NURSE as soon as possible

BEFORE the deadline. Thank you.

Hard copies of PPE Medical Eligibility Forms are due to the Nurse by October 29, 2024.

****** WINTER SPORTS BEGIN ON NOVEMBER 13, 2024 ******

If your student wants to play a WINTER sport or intramural,

the Date of Exam must be 11/14/23 and later.

HELPFUL HINT #1: ***Please check your child's sports physical exam date on the Genesis Parent Portal. If there is NO DATE, then your child's sports physical is EXPIRED. You must submit a NEW PPE Medical Eligibility Form.

HELPFUL HINT #2: Sports Physical PPE Medical Eligibility Forms are valid for 365 days from the Date of the exam.

Parent/Guardian must also complete the following forms in Genesis for a student to be eligible to participate in a WINTER sport or intramural:

• WINTER Health History (Seasonal)
• Concussion Release (Annual)
• Sudden Cardiac Release (Annual)
• Opioid Fact Sheet and Video and Release (Annual)
• Sports Related Eye Injuries (Annual)
• WINTER consent for Athletic Participation (Seasonal)

HELPFUL HINT #3: Complete all your Genesis Sports forms as soon as they are published in the portal. ***You may have more success if you use a computer instead of your phone.

HELPFUL HINT #4: The NEXT DAY check to be sure all your forms submitted on the portal. Promptly report any discrepancies to the Nurse.

***No WINTER paperwork will be accepted once the WINTER season begins on

November 13th.
