Medical Forms for School Nurse
Please review the list of medical forms in this folder.
If your student takes medication at school, requires an Emergency Action Plan, or has any other unique medical needs, please take the pertinent form(s) to your student's medical provider to complete them before the start of the school year.
Maryland Sun Bucks
Maryland SUN Bucks helps families with school-aged children who receive free or reduced-price meals buy groceries during the summer break when school meals are unavailable. Families who qualify for the Maryland SUN Bucks program will receive $40.00 for June, July, and August (a total of $120.00) for each eligible student.
You may be eligible for Maryland's Summer 2024 SUN Bucks. Most eligible children are automatically enrolled, but some families will need to apply. Fill out a short SUN Bucks application here. For more information about Maryland SUN Bucks, select here.

Avoid the crowds and lines! Teachers and staff will be available to meet students and families throughout the entire 90 minutes of Open House. A later arrival may mean less time in line!

FREE Summer Meals
5-Day Summer Meals Pickup
The SMCPS Food & Nutrition Services Department is providing a 5-day pickup for summer meals. These meal bags include five breakfasts, five lunches, a half-gallon of white milk, and two cartons of chocolate milk for each child.
Meal pickups are on Mondays with two pickup times for your convenience. 11 am - 1pm or 4pm - 6pm. The schools offering pickup are:
- Chopticon HS
- Leonardtown HS
- Oakville ES
- Spring Ridge MS
Parents/Caregivers, if you did not sign-up ahead of time, you must have your student(s) in-person for meal pickup.

Free Back-to-School Vaccine Clinics
The St. Mary’s County Health Department (SMCHD) is hosting free back-to-school vaccination clinics for children ages birth to 18 years living in St. Mary’s County. Appointments are recommended. The following vaccines will be available:
- Tdap (tetanus, diphtheria, whooping cough)
- Meningococcal (meningitis)
- MenB (Serogroup B Meningococcal)
- HPV (human papillomavirus)
- Varicella (chicken pox)

Kindergarten Registration - Open registration for students who will be five years old by September 1, 2024 continues throughout the summer.
SMCPS Registration Information
Required documents include:
- Proof of residence
- Evidence of Birth
- Immunization Record
- Court papers regarding custody (if applicable)
- Photo ID of parent/guardian
Out-of-District Transfer Requests
- In order to attend LMDES, valid proof of residence must be provided
- If you do not reside within LMDES district boundaries, you must submit a transfer request (annually). This does not guarantee placement.
Pre-Kindergarten Application
St. Mary’s County Public Schools (SMCPS) offers full-day Pre-kindergarten 4* to children who reside in St. Mary’s County Maryland; meet age requirements; and are income eligible.
- Eligible students must be 4 years old on or before September 1, 2024.
- All Pre-Kindergarten classes for 4-year-olds are full day.
- Acceptance is not First-Come, First-Served; it is determined by a tiered eligibility system.
- Income verification is required of every applicant.

*Lettie Dent has achieved a Maryland EXCELS check LEVEL- 5 rating by the Maryland State Department of Education (MSDE). Maryland Excels awards ratings to child care and public pre-kindergarten programs that meet increasingly higher standards in key areas that are identified nationally and contributing to high quality. Level – 5 is the highest rating.
