Welcome to Creekwood Middle School, home of the Colts. We are excited to have you join us for the 24-25 school year. Creekwood is a wonderful community of caring and supportive staff, students and families that work together to support students in their academic growth and their social emotional needs. Being a Creekwood Colt means embracing and living our core values every day. It is about building a strong community where everyone feels connected and valued, taking ownership of our actions and learning, demonstrating leadership by inspiring and supporting others, and showing tenacity in the face of challenges. It is the COLT Way, and we are looking forward to having you join our family. Welcome Colts!
We are looking forward to meeting all of our Creekwood Colts this year. Before our school year begins on August 7, all CMS students MUST attend our Virtual Colt Camp. We ask that everyone complete their Virtual Colt Camp by August 5, 2024. Before you begin, please download the Colt Camp Checklist to help you prepare for camp. We will see you soon!
What documentation will I need to upload into HAC?
- Immunization Records - must be SIGNED or STAMPED by the physician or his/her designee or public health personnel (including Mychart records)- typically for 7th grade, but required during a child's 11th year of age. Proof of residency in Humble ISD - Must complete by August 2.
* For students residing in a house, the lease, the deed, or a current utility (water, gas, or electricity) bill is required.
* For students residing in an apartment, a lease containing parent and student names or a letter from the apartment manager is required.
* If both parent and student live with a relative or friend in Humble ISD whose residence is not an apartment, the parent and the person with whom they reside must submit a notarized address affidavit form that must be picked up from CMS, the forms are only available at the campus. Please note, when presenting this form a meeting with the campus principal, the parent/guardian, the student, and the homeowner is required. Forms can be picked up from the CMS Registrar, Ms. Jackson. Due to Creekwood being under constructions, Ms. Jackson is currently officing out of Kingwood Middle School until August 1, 2024.
Step #1: HAC is NOW OPEN- Complete the Annual Update (REQUIRED)
Students who were newly registered to Humble ISD will not need to complete this step. Annual update is for returning students only.
Once the email is received that an Annual Update is available for a student, a parent can complete and submit the form in the Home Access Center (HAC). For more information go here, Humble ISD Annual Update Information.
Steps for completing the Annual Update:
- Log in to Home Access Center and check the student name in the top right corner. Select the drop-down circle to change to a new student, if needed.
- From the Registration page, select the Update Enrollment tab.
- Select Start to access the Annual Update Registration.
Step #2: RevTrak Online Purchases
Orders will be filled in a timely manner and sent home with the student. Thank you.

All Creekwood students are encouraged to have a planner or calendar to keep track of upcoming assignments and tests.
Additional ID Badges - The first badge is free, but this link will allow you to buy extras (optional). We encourage all students to have at least one or two extra ID badges. It is required to have a badge on and around your neck visible for our staff to identify students. If you do not have one you will be required to obtain a temporary ID badge, and there are consequences for students who have to use the temporary badge after they use their one time free temporary badge.
Spirit Merchandise - Purchase spirit shirts, sweatshirt, hoodies, lanyards, string bag, and all things CMS (optional). Each week students are encouraged to wear their Creekwood spirit shirts for Creekwood Spirit Day! You can never be too spirited when you are a Creekwood Colt!
Purchase PE or Pre-Athletics workout clothes. Every student needs one shirt and one pair of shorts, but two pairs of each are recommended (CMS Workout athletic clothes are required for those registered for PE or Pre-Athletics).
CMS Girls' Athletics Grays - one pair of shirt and short are required but two are recommended (Required for those 7th or 8th-grade girls signed up for Athletics).
CMS Boys' Athletics Grays - one pair of shirt and short are required but two are recommended (Required for those 7th or 8th-grade boys signed up for Athletics).
CMS Dance Shirt for students enrolled in the dance class.
Step #3 Set Up Lunch Accounts
Step #4 7th grade and 8th Grade ONLY - Upload Physicals and Complete Rank One
If your child will be participating in athletics, cheer, drill team, or high school band (Not PE) during the 2024-2025 school year, you will need to complete an Emergency Card, sign the Humble ISD/UIL Rules, and, if playing football, complete the Helmet Form. These forms, along with a current physical on file (dated after Feb. 1st, 2024) must be completed prior to being allowed to participate next school year. All physicals must be completed on the Humble ISD Physical Form. Click here for a link to the form.
The forms may be found online at http://tiny.cc/humblesport . It is recommended to complete the forms as a guest rather than making an account.
Additionally, if your child has a medical condition that necessitates the use of medication (asthma, diabetes, seizures, severe allergies, etc.) it is required that they have their medication at every practice or game. Please be prepared to provide necessary medications to their athletic trainer/coach before they will be allowed to participate. Medications may be picked up by the student following the end of the season or school year. This requirement is for the safety of your child as athletic trainers or coaches do not have any emergency medication. Paperwork regarding the medication must be on file with the school clinic.

CMS Annual Fund
Click here to support our students and teachers with a donation to the CMS Annual Fund.
The CMS Annual Fund is a joint effort between Creekwood Middle School, the CMS PTA and the Humble ISD Education Foundation to raise funds to help us achieve our academic vision. With our Annual Fund, we have been able to enhance technology for students, provide a more extensive Positive Behavioral Interventions and Support program, provide flexible seating in our classrooms and other learning areas. Annual Fund gifts have also allowed us to send teachers to professional development opportunities giving them the training they will use that directly benefits our students. This next year we are working on the modernization and enrichment of our campus library. The library serves as a critical resource center for our children, fostering a love for reading, research skills, and academic achievement. With the additional flexible learning areas and acquisition of new books, including literature that reflects our students' backgrounds and interests we will aim to create a vibrant hub for research, creativity, and intellectual exploration while continuing to focus on our continuous improvement of academic outcomes.

If supplies were pre-ordered by July 1, 2024, they will be delivered directly to your home address.
Creekwood has recently been remodeled and we no longer have hallway lockers for our 6th grade students. Please make sure that your student has a backpack to use throughout the day to transport their binders and materials.
If you are enrolled in PE, Pre-athletics, Athletics, or Dance you will need to provide your own combination lock for the locker room lockers. We suggest purchasing early and practicing before school starts.

Drop Off /Pick Up for Regular and Inclement Weather with Map
Please take some time to review the Creekwood Middle School Drop Off and Pick Up Procedures. Please review the Map of Traffic Flow Procedures
· 7:45 AM - Doors open on the commons side (Sandy Forks Side) and 700 hallway (West Lake Houston Side) for students to enter the building.
· 8:25 AM – First bell rings for students to go to 1st Period, and the commons doors are closed and secured. Students arriving between 8:25 and 8:30 should enter through the main entrance vestibule on the Sandy Forks side of the building.
· 8:30 AM – Tardy bell rings. Students arriving after the bell must enter the building through the main entrance vestibule on the West Lake Houston side.
· 4:00 PM - Students are dismissed
· 4:10 PM – all students should be out of the building or in their place of tutoring, practice, or club meeting.
· For students who need to call home, a phone is available in the main office.
