Note From The Nurse
Reminder - 11th-GRADE PHYSICALS Are Due
The Pennsylvania school code mandates medical examinations for students in grades K, 6, and 11. Physicals are to be completed within 4 months prior to the start of the school year. Contact Nurse Jones @ s.jones@upperadams.org if you have any questions. You may pick up a physical form in the school office or by selecting the link provided Private or School PHYSICAL EXAMINATION FORM.
MEDICATION POLICY REMINDERS:
- If your child needs medication during school hours, this includes carrying and self administering inhalers or EPI PEN's , please have their doctor complete a medication administration form. The form can be found on the district website under forms or please stop by to pick one up from Nurse Jones. Completed forms are to be returned to Nurse Jones.
- Prescription Medications: The school will administer only those medications supplied by the parent or guardian. It is the responsibility of the parent to ensure that the medications are properly delivered to the school. Parents are strongly encouraged to bring these medications to school. Prescription medications must be delivered in a pharmacy or physician-labeled container (child-proof, if available).
- Medications
- The label must contain:
- The student’s name;
- The name of the physician;
- The date of the prescription;
- The name and telephone number of the pharmacy
- The name of the medication, dosage, route, and frequency of administration.
- Documentation: A completed Upper Adams School District Administration of Prescription Medication Form, signed by the doctor, and a Parent or Guardian Release and Waiver Regarding Administration by School District of Medications and Medical Treatment to Student form signed by the parent or guardian must accompany the properly delivered medication. These documents will be valid for the current school year only. The school will not administer any medication unless the required forms have been turned into the school and have been properly completed.

REMINDERS
STUDENT PROFILES
Every year, parents and guardians are required to update student profile forms. This applies to both new and returning students.
Student Profile forms ensure important student information -- such as guardians, emergency contacts and medical information – is up-to-date allowing the district to respond appropriately in case of emergency. Maintaining up-to-date student information is also needed to maintain compliance with the UASD. There are two sides to the form--the front side contains student's demographic information and the back side, permissions for OTC medication, emergency contacts, and school notification system options. Even if there are no changes, all forms must be signed, dated, and returned.
A student packet containing the profile and other important information was given out to returning and new students on the first day of school in their advisory period. We encourage families to complete their forms immediately.
UASD Annual Technology Device Protection Plan
This is a reminder that computer device forms are due. Students were given technology forms for their laptop device at pickup. All forms must be turned in to the office immediately. Students can either opt to go with the protection program for a fee of $30 or waive out of the program. Should you decide to waive out of the program, a parent/guardian must sign the form that you understand the terms and will be responsible for the payment of repairs or replacement of your student's device should the device get damaged. Please select links below to view forms. You may pay by cash, check (made out to UASD), or MySchoolBucks.com.
ACADEMICS
Language Recognition
UASD and Biglerville High school, under the mentorship of Maestra Pecher, would like to congratulate the following students for meeting the requirements to receive the The Seal of Biliteracy and The Global Seal of Biliteracy.
- The Seal of Biliteracy is a distinction presented by a school, district, or state to acknowledge students who have achieved proficiency in two or more languages by the time they graduate from high school.
- The Global Seal of Biliteracy certification aligns with or surpasses the criteria of all current U.S. state Seal programs, providing a consistent benchmark for acknowledging language proficiency levels relevant in academic and professional settings. This certification enables individuals to demonstrate their language abilities to educational institutions and employers worldwide, transcending state boundaries and international barriers.
Maestra Pecher, "I am so grateful for your hard work and dedication. It has paid off!"
