Winter Sport In-Take Days
Each athlete is required to hand deliver their completed, and accurate paperwork to our school nurse.
The nurses’ office will be accepting sports forms throughout the school day. Students should first report to their classes and then come down with a pass. Teachers will notify the nurses when a student is on their way.
Please understand that the nurses’ office prioritizes students with medical needs; if you are asked to return later, we appreciate your cooperation. We also kindly request that parents avoid calling to check on form submissions, as this can slow down the process. Thank you for your understanding and support!
In Order to Participate:
âś…The MOHS or MOMS School Nurse must receive all signed medical forms (No electronic submissions will be accepted).
Hand your forms to the NURSES office. You must hand deliver.
To PARTICIPATE:
âś… Have completed and doctor-approved Annual or Physical Update Form
âś… Hand your forms into the nurse's office
âś… Registered for their sport via the Online Registration Portal
âś… Received final clearance status from our school doctor
Medical Forms and Examination
All students who participate in the athletic program are required to obtain a physical examination as per NJ state law. A new physical is required every 365 days:
*IF YOU HAVEN’T HAD A PHYSICAL WITHIN 365 DAYS: THEN YOU WILL NEED A FULL Winter Pre-Participation Physical Exam
*IF YOU HAVE HAD A PHYSICAL WITHIN the 365 DAYS AND HAVE ALREADY SUBMITTED YOUR MEDICAL ELIGIBILITY FORM, THEN YOU WILL ONLY NEED A Winter Health History Update Form

To participate on a school-sponsored interscholastic or intramural athletic team or squad, each student whose physical examination was completed more than 90 days prior to the first day of official practice shall provide a health history update questionnaire completed and signed by the student’s parent or guardian.
Forms can be picked up in the Athletic Office or printed from the website/above.
ADDITIONAL MEDICAL FORMS, IF APPLICABLE:
**If you have Asthma, a life-threatening allergy or other pre-existing condition, you will need to update the MD orders for the school year.**
**Students will not be eligible to practice without current Doctor’s orders.*

Homeschool Athletic Eligibility Criteria
Policy 9270: Homeschool and Equivalent Education Outside the Schools
A course of study and transcript will need to be reviewed with the High School Administrative team 30 days before the beginning of the official NJSIAA start date for that season.
