One-Time Immunization Extension
ACTION REQUIRED: Get Updated Vaccines by September 3rd!
Vaccines Are MANDATED By State Law!
Need more time getting your student's required immunizations? The West Virginia Department of Health (WVDH) has granted Berkeley County Schools a two-week extension to obtain all immunizations required for school entry.
The Berkeley-Morgan Health Department (BMHD) is offering appointments to assist your student with getting their required vaccines. Please use the link below to schedule an appointment; do not call their office directly.
Shenandoah Community Health offers student vaccinations at their walk-in clinic, HealthExpress, at 99 Tavern Road from 8 a.m. to 3 p.m. Monday-Friday.
Please submit your student's updated vaccination records directly to your school. If you have already submitted an updated copy of your student's vaccination records, no action is needed.
Thank you to the West Virginia Department of Education and WVDH for their leadership in allowing this extension which will ensure uninterrupted instruction while required immunizations are obtained.
Pre-K Entry Requirements
All children entering an approved Pre-K classroom must have age appropriate immunizations upon enrollment as mandated by state law:
DTP/ DTap: 4 Doses
POLIO (IPV): 3 Doses
Measles, Mumps, Rubella (MMR): 1 Dose (after the 1st birthday)
VARICELLA: 1 Dose (after the 1st birthday)
Hepatitis B- Series of 3 (last dose must be after the age of 6 months)
K-12 Entry Requirements
DTaP/ DTP/Td/Tdap: 4 Doses (one dose MUST be after the 4th birthday)
POLIO (IPV): 3 Doses (One dose MUST be after the 4th Birthday).
Measles, Mumps, Rubella (MMR): 2 Doses (first dose MUST be after the 1st birthday)
VARICELLA (or hx of Chicken Pox): 2 Doses (first dose MUST be after the 1st birthday)
Hepatitis B- Series of 3 (last dose must be after the age of 6 months)
7th & 12th Grade Entry Requirements
ALL children enrolling school in grades K-12 MUST have age appropriate immunizations as mandated by West Virginia State law PRIOR to attending school!
All students entering 7th Grade must show proof of immunization against TDaP, and meningitis:
- TDaP: Proof of one dose of TDaP vaccine.
- MCV4: Proof of 1st dose of MCV4 vaccine.
All students entering 12th Grade must show proof of immunization against TDaP, and meningitis:
TDap: Proof of one dose required. If not obtained at age 11 or older.
MCV4: Proof of 2nd dose of MCV4 if first dose was received before the 16th birthday.
