Title VI Indian Education
Title VI
- The original Title VI ED 506 Indian Student Eligibility Form is required for each student to be placed on the program.
- Title VI provides educational and culturally related services for those students enrolled in the Edmond Public School district.
- Please send original forms to the Indian Education Office where they will be housed for federal monitoring purposes.
- After a completed form is verified by the office, the student will be flagged on Infinite Campus/EPS Indian Ed.
Student Eligibility
- Information needed for the form to be complete may be from the student, parent or grandparent.
- Should tribal enrollment information not be readily available, contact the Indian Education office for assistance.
- A list of Oklahoma tribal address's is available upon request.
- You may contact our office should a student be from a tribe outside the state of Oklahoma.
- A copy of tribal documentation is greatly appreciated but not required for the Title VI program.
- However, a copy of the student's tribal documentation is required for the Johnson O'Malley Program. Please contact the office for additional information on the JOM program.
- A Title VI ED 506 form received and verified for the Title VI program places the student on the program until they move or graduate. A form is not necessary every school year.
