Upper Moreland High School is partnering with AFC Urgent Care in Willow Grove for a chance to get your PIAA sports physical for the 2024-2025 school year.

WHERE: Upper Moreland High School Nurse's Suite
COST:
$25 cash or check made payable to AFC Urgent Care.
Payment is required at the time of the physical.
REQUIRED FORMS:
Report to the High School Main Office with Section 5 COMPLETED, and Section 6 BLANK.
Sections 5 & 6 of the Physical Form must be dated and signed by the physician at your appointment.
*PIAA requires physicals to be updated yearly after May 1st of each year.*
