The Yellow Sheet
The Yellow Sheet -- September 6, 2024
This Yellow Sheet (YS) is pretty long and definitely can be skimmed. There is a lot of information that schools are required to provide to families at the beginning of each school year. We have successfully finished the first week of school!

OMNY
NYC Public Schools provide our students with OMNY tap-to-go passes for subway and city buses. LaGuardia distributed OMNY cards on the first day of school. This will allow eligible students to get to and from school and school-related activities for free—and ride for free on weekends and all year, too! (The cards work through August if still enrolled in NYCPS.)
Getting help with Student OMNY Cards
Students who still need their OMNY Cards -- or need assistance -- should go to Room 230, Main Office.
About Student OMNY Cards
Student OMNY Cards are:
- Distributed exclusively by your school. You can’t get a Student OMNY Card from the MTA.
- Good for 4 free rides each day. You can tap a Student OMNY Card whenever you want to take up to 4 free trips every day of the week, any time of the day.
- This includes a free transfer between the subway and local, limited, and Select Bus Service buses, or a free transfer between buses.
- Valid all year long. Your Student OMNY Card doesn’t expire until the next school year.
If you’ve used Student MetroCards in the past, you should know that Student OMNY Cards give you more free rides with fewer limits. Student OMNY cards can be used:
- All day (not just certain hours)
- Every day of the year (not just school days)
- Every day of the week (even on weekends and over the summer)
- All year long (your OMNY card is valid for the entire school year and the summer)
Your Student OMNY Card is for you—and only you! Don’t share it and risk having your pass confiscated. Remember to always tap for your rides. Your tap tells the MTA how many people ride from every station and helps improve service.
ID Card Replacement
All students are required to carry their LaGuardia HS ID Card on them while in the building.
Open Lunch Program For Grades 10, 11, and 12 (Begins September 17)
The first day students may leave the building for Open Lunch will be Monday, the 17th. This will allow for processing all forms and updating the scan machines. If we are able to begin the program earlier – such as Thursday or Friday – we will let families and students know the day prior to students leaving the building.
Open Lunch Expectations
Students are expected to submit a copy of their signed Open Lunch Consent Form.
Students are expected to re-enter LaGuardia at least 5 minutes before the start of the following class period - remember to account for potential delays in re-entering the building.
Students are expected to arrive at their following class period on time.
Students are expected to swipe their ID card on the CAASS machine when exiting and returning to the building.
Students are expected to remain in the general neighborhood. Staying within a few blocks of LaGuardia will ensure a safe return to the building.
Students are expected to exercise sound judgment while off-campus, behaving respectfully in the neighborhood and representing LaGuardia positively.
Students understand that Open Lunch is a privilege that can be revoked if they do not follow the guidelines.
Once a student has left the building, LaGuardia staff cannot supervise the student's safety. If students have difficulty meeting Open Lunch expectations, they put themselves in unsafe situations. If a student does not meet the expectations of Open Lunch, Open Lunch is an unsafe option, and it will no longer be available to the student.
Printed copies to be brought home and signed are available outside Room 229; families may print the Consent Form.
Signed Consent Forms should be returned to Room 229 (Deans Office).
Lunch Update
The Cafeterias on the 5th and 7th Floors may have felt a little crowded these first two days of school. This situation is only temporary and will resolve itself soon. Once the Library opens, Open Lunch begins, students can access Practice Rooms in the basement, Open Studio in Fine Arts classrooms, and small group instruction at various places in the building, the cafeteria will have significantly fewer students each period.

It is the policy of the Department to maintain a safe and supportive learning environment that is free from harassment, intimidation and/or bullying and from discrimination on account of actual or perceived race, color, citizenship/immigration status, religion, creed, national origin, disability, ethnicity, gender, gender identity, gender expression, sexual orientation or weight. The policy prohibits such behavior by students against other students and by staff against students.

Such discrimination, harassment, intimidation and/or bullying is prohibited in school, during school hours, before or after school, while on school property, at school sponsored events, or while traveling on vehicles funded by the Department of Education.
It is also prohibited on other than school property when such behavior disrupts or would foreseeably disrupt the educational process or endangers or would foreseeably endanger the health, safety, morality or welfare of the school community.
Families and students may receive printed copies of s of Chancellor’s Regulations A-831 (Student-to-Student Sexual Harassment) and A-832 (Student-to-Student Discrimination, Harassment, Intimidation, and/or Bullying) if they do not have a way to access to the electronic versions. Military/Institutions of Higher Education Recruitment Opt-Out Notification
Federal law requires the New York City Public Schools (NYCPS) to provide the following information about 11th and 12th grade high school students to military recruiters and institutions of higher education that request this information, except where the parent of a student who is younger than 18 years of age, or a student who is age 18 or over, opts out by notifying the DOE in writing that they do not consent to release this information. If the student is under age 18, the parent must sign the opt-out. If the student is age 18 or over, the student must sign the opt-out.
- name, address, and telephone number to institutions of higher education that request this information and/or
- name, address, telephone number, and DOE provided student DOE email address to military recruiters who request this information. (Please note that any emails sent by military recruiters to student DOE email addresses are not controlled, sponsored or reviewed by the DOE.)
While we are committed to protecting the confidentiality of our students, we must comply with the law.
If you are a parent of a child younger than 18, and you do not consent to the disclosure of this information, you must complete an opt out form (either electronically, through your NYCSA, or use your camera to open the QR Code) to your child’s school by October 18, 2024. If you do not return the form by this date and your child is a student in the 11th or 12th grade, NYCPS will release your child’s information upon request. For parents of 9th and 10th grade students, the opt-out form can be completed and saved in advance.
If you are a student who is 18 years of age or older, you must determine whether to consent to release the information. Students 18 or older who do not want to release their information must complete, sign, and submit this form to their school by October 18, 2024. If the opt-out letter is not returned, the student’s information will be released upon request.
Parents of students younger than 18 and students ages 18 and older who do not complete a form now may do so at any time the student is enrolled in a DOE school. For more information or assistance, please contact your school. Thank you for your cooperation.
CATCH Program
OFFICE OF SCHOOL HEALTH
Dear Parents and Guardians,
Your child’s health is important to us. Unintended pregnancy is a concern for New York City teenagers. About a third of New York City public school students become sexually active before completing high school. In 2021 in New York City, over 1,300 females under the age of 18 and over 3,400 females 18 and 19 years old became mothers or terminated a pregnancy. Most of these pregnancies were unintended. Most teen mothers do not graduate from high school with their class.
To provide our high school students with timely and accessible services the Office of School Health began a reproductive health program in 2011 called CATCH (Connecting Adolescents to Comprehensive Healthcare). This year the CATCH program will be offered at your child’s High School. Services will be provided by the School Health nurse and a School Health doctor or nurse practitioner. The program will include reproductive health education, including information on sexually transmitted infection prevention. The program will also include reproductive health services: pregnancy testing
(urine testing, no physical exam will be required), and hormonal contraception (birth control), including emergency contraception and condoms. Students must sign a form consenting to these reproductive health services before receiving them. All services are confidential and free of charge.
In New York, minors may receive reproductive health services without parental consent. Because CATCH services are being provided in school, parents may choose not to have their child receive the services (“opt their child out”) by returning the bottom of this letter to the principal. If you do not choose to opt your child opt, your child may access these reproductive health services if they choose. Please note that parents may not opt out from receipt of reproductive health services students who are 18 years of age or older, students who are parents or legally emancipated, or students in certain
other circumstances, such as in emergency situations. Please be aware that all students will have access to health education and referrals for reproductive health regardless of whether their parents choose to opt them out of reproductive. health services. We encourage all parents to talk to their children about their reproductive health as a part of their overall health.
It is our hope that this program will provide a chance to educate our students about pregnancy prevention and help them make responsible choices. If you DO NOT want your child to participate in any of the reproductive health services offered in connection with the CATCH program (pregnancy testing (urine testing, no physical exam will be required); and hormonal contraception (birth control, including emergency contraception and condoms) please complete each field in the Opt Out formbelow AND SIGN this portion and return it to Assistant Principal Stricklin-Witherspoon (Room 230 Mailbox, Main Office) in an envelope marked “CONFIDENTIAL.”

Name and Gender Change Request Form
Dear Parent or Guardian,
When you enroll your student in a New York City Department of Education (DOE) school, we are required by law to collect information about your student’s gender. Please complete this form to select the option that best describes your student’s gender. We understand that the options we provide may not fully represent your student’s gender identity. We will enter the option you select into our student information systems. If your student uses a different first and/or last name than the name that appears on legal documentation, you can also provide a chosen name using this form. The school community will address your student using the name and pronouns your student asserts at school.
DOE schools do not include gender on student- and school-facing permanent records. You may update your student’s gender in the limited places where gender is recorded by submitting a signed Name and Gender Change Request Form to your school at any time. You can also use this form to request changes to your student’s chosen name if you provide one. Students who are emancipated minors or 18 years of age or older should sign and submit this form to request these changes.
If your student changes their legal name through a court order, amended birth certificate, or government ID, submit that documentation to their school to update their legal name on their permanent records. DOE schools maintain a safe and supportive learning environment for students of all gender identities. For additional resources for supporting transgender and gender expansive students, see schools.nyc.gov/lgbt.
Know What's Going On
Documentary
During this upcoming 2024-2025 school year, a small film team will be on campus in select classes, working on a documentary project celebrating LaGuardia High School, as the school moves into its 40th anniversary year. Filming will be limited to only a few days per month and will be at times that teachers feel are not disruptive to those particular classes. The film team is focusing on this year's senior class. We know there may be parents who have concerns or if students do not wish to participate. No one will appear on screen unless explicit consent is given. (We are just informing families of what is happening. Once the project is further along and there is a better idea of who may appear in the film, then consent forms will be issued.)
The filmmakers' "A REVOLUTION ON CANVAS" can be seen now on HBO/Max. Visit www.partnerpictures.com for more info or IG: @arevolutiononcanvas
