Below you will find links to two important forms that we must have on file. Please let us know if, after reading through the linked information, you have questions.

The following items must be completed by August 19:

• Emergency Card (linked below) for ALL STUDENTS, Pre-K through 5th Grade

• After School Care Contract (linked below) for ALL Kindergarten through 5th Grade Students

All Pre-K through 5th grade families must complete an Emergency Card for each child enrolled at Saint John School. This can be completed one of two ways: Digital or Hard Copy.

DIGITAL:

• download the fillable PDF linked below,
• complete one for each of your Saint John children, being sure to sign all three highlighted locations,
• email to Mrs. Wendt at twendt@saintjohnsch.net

HARD COPY:

• download and print PDF linked below,
• complete one for each of your Saint John children, being sure to sign all three highlighted locations,
• mail/bring the original copy with all three signatures to the office

Please do not return unsigned Emergency Cards via email.

There is a signature line on the Emergency Card that indicates you have received and read the Parent Student Handbook. This is linked below.

All Kindergarten through 5th grade families must complete the After School Care Contract linked below, regardless of whether or not you plan to use the After School Care Program. Emergencies occur when last minute After School Care use might be necessary.
