Join us during the month of January for 30 days of gratitude. Simply print the form to the left and write in one thing you are grateful for each day. On January 31 turn in your form to Kim Smith for a chance to win a wellness prize.
Live Healthy Iowa: 10 Week Wellness Challenge
Cost: FREE ($25 fee will be paid for by the UHC wellness budget)
Over the course of 10 weeks, teams (2-10 people) track activity minutes and/or weight loss through the Live Healthy Iowa website. This simple and affordable challenge provides Iowans an opportunity to improve their health while engaging in fun, friendly competition.
Participants receive:
- Challenge T-shirt (Adult small - 4XL)
- Personal online dashboard with the ability to sync Fitbit devices and Under Armour apps
- Complimentary subscription to Fresh Pickings magazine
- Weekly motivational messages

Try the Y - Free Passes
The Norm Waitt Sr. YMCA has provided the District with a number of "Try the Y" for free passes. If you have interest in receiving one please email Kim Smith.
Early Retirement - Don't forget the submission deadline is January 15
The Early Retirement Policy and application forms are linked below. You must meet all three of the following requirements to be eligible to participate:
· Age 62 or older by June 30, 2022, AND
· 25+ years of service with our District, AND
· Eligible to begin retirement through IPERS
Eligible employees who meet all three conditions will need to submit a Retirement Notice and Application for Early Retirement to Human Resources by January 15, 2022, to Kim Smith, Insurance and Benefits Manager. Any years worked part-time, but was at least 20 hours weekly, will be counted as one-half year of credit.
You may check your accumulated sick leave days in the Employee Access Center.
Regular Retirement
For those looking into retirement that may not qualify for the Early Retirement policy, you must complete both the District's retirement notification form AND the IPERS employment verification form.
- IPERS employment verification form can be sent to Arika Irvine, Payroll Supervisor.
- District retirement form can be sent to Kim Smith, Insurance and Benefits Manager.
Do you need to update your IPERS or Life Insurance Beneficiaries?
- Print the forms below, complete, and return to HR.
NOTE: If you are married, your spouse must also sign the IPERS form.
