
Exploring Transition Discoveries
2024-2025
Transition Discoveries is a research-based model designed to improve current transition practices. This training provides an overview of transition and looks at resources available through the updated Transition Discoveries program. Young people with disabilities, families and professionals who support them make up a powerful network of relationships, knowledge and experience for what's working in transition. Through the power of collaboration, the Transition Discoveries initiative connects these networks into a detailed road map for success.
