Tuesday Transition Tips
Transition Tip #35
Informing Parents, Guardians, and Caregivers About Postsecondary Transition
Transition planning for students with disabilities is an important process that helps them prepare for life after high school, whether they plan to attend college, enter the workforce, or pursue other goals. The process can feel overwhelming, especially for parents, guardians, and caregivers, who may already be managing other challenges in their lives. Understanding the role of parents/caregivers/guardians in transition planning, and having access to helpful resources, can significantly ease the burden and lead to a smoother experience.
Here are two valuable resources that provide a roadmap for supporting students and families in transition planning:
START Project: Making the Most Out of Transition : A Brief for Families
Michigan Alliance for Families Information, Support, and Education
Now is a great time to remind parents/guardians/caregivers that the Washtenaw Intermediate School District (WISD) is offering valuable resources and events to support parents, caregivers, and guardians in the transition planning process. These events are excellent opportunities for families to learn about the transition process and gain the knowledge they need to effectively navigate postsecondary planning for their children.
Here’s a message you can share with parents and caregivers about upcoming WISD postsecondary transition events:

Why These Events Matter:
Transition planning can seem distant at times, but the earlier you begin to plan and gather information, the better prepared you and your child will be. These events provide an invaluable opportunity for parents/guardians/caregivers to ask questions, connect with local resources, and ensure they are on track to support their child’s postsecondary goals.
We encourage all parents, guardians, and caregivers to mark these dates on their calendars and make plans to attend these important events. Your participation will help ensure that you are equipped with the knowledge to advocate for your child’s needs and assist with creating and implementing a successful transition plan for their future.
Community Agencies and Supports in Washtenaw County:
WISD also collaborates with various community agencies in Washtenaw County that can provide further support for your child’s transition. Stay informed about the services available to you and your child through the WISD Community Agencies and Supports. These resources can help with job training, independent living skills, postsecondary education, and more.
We hope to see you at these events! Your involvement is crucial to your child's success in their postsecondary journey.
Please share this information with your school community and encourage them to attend these important events. Having this information early allows families to plan ahead and take full advantage of the support available to them.

Washtenaw County Post-Secondary Transition Manual
Washtenaw County Post-Secondary Transition Manual (Fall 2023 revision date)
This Tuesday’s Transition Tip is a review to some and introduction to others. It is important as special education providers/case managers that we create structured and detailed postsecondary transition plans for our students. The Washtenaw County Post-Secondary Transition Manual is a guide to assist special education staff with creating complaint post-secondary transition plans. This guide contains a table of contents to help locate information quickly. The manual also gives insight on how not only to build a complaint transition plan but a quality transition plan. It includes links, videos, and an appendix to assist. Please take the time to read and review the Washtenaw County Post-Secondary Transition Manual.
Don't forget our monthly Countywide Transition Coalition Meeting for Washtenaw County Transition District Designees, WISD teachers, consultants, and service providers is held on the 2nd Wednesday of every month from 9am-11am at the Teaching and Learning Center.
