Practices for the 2024-2025 OHSAA sports season officially begin on the dates listed on the Future OHSAA Sports Dates Webpage. We will continue our partnership with FinalForms, an online forms and data management service. FinalForms allows our Athletic Families to complete and sign Athletic Forms for your student-athletes. You can start the process here on our Perry Athletic Website.

Here is a short video that shows you how the Athletic Registration Process Works, from Parents to School Staff once you have set up your account. All student-athletes and parent/guardians MUST have their Athletic Registration completed and their 24-25 Sports Physical uploaded to FinalForms or turned into the Athletic Director, Seth Evans, in order to participate with their sport on the OHSAA official tryouts/practices dates.
