Health Requirements & Forms for the 2024-25 School Year
If you have already submitted your students' current health exam and physical, current and up to date immunizations form and dental forms, please disregard.
A reminder for 6th grade parents: All 6th grade physical and immunizations (including Tdap and Meningococcal vaccines) are now due. If you recently had an appointment for a physical and/or immunizations please submit all health documents as soon as possible to remain in compliance. This includes verification of immunizations as well as physical, dental examinations. Illinois law also requires that proof of an eye examination by an optometrist or physician (such as an ophthalmologist) be submitted to the school for all students who are enrolling in an Illinois school system for the first time. You may drop off all forms to the school. If you choose to email the forms, please keep in mind that you must send a clear and well lit photo or scan of the documents to be printable.
7th & 8th Grade parents: If your middle school student has an updated physical, please submit to the school nurse. If your student will be playing an intramural sport, a sports physical will need to be completed if we do not have a current 7th or 8th grade physical on file completed within one year prior to the first day of the current school year.
Please contact the school nurse, with any questions, Ana Rivera-Escobar at 847-859-8608 or email: riveraescobara@district65.net

2-minute Survey About D65 Grading
Over the past several years, District 65 has been in the process of implementing a standards-based learning and grading system. In December, Chute students along with other D65 middle schoolers will receive report cards that reflect this new system.
Before September 12, please take two minutes to help us understand your level of awareness and understanding of standards-based grading. This information will be kept anonymous.
Please disregard if you have already taken the survey. We ask that it be completed only once. Thank you for taking the time to share your perspective!

SAEBRS & my SAEBRS
More than two decades of research have demonstrated the important role that social and emotional skills play in each child's success in school and in the community. Skills such as demonstrating self-control, persisting on challenging tasks, and making good decisions are all necessary to be successful students and adults. The process by which students learn these and similar skills is called social emotional learning (SEL). As part of our efforts to better support our students in developing positive social and emotional skills, we are using the SAEBRS SEL assessment for all students in grades K-8.
The SAEBRS assessment is a brief 19-question survey answered by your child's teacher for students in Kindergarten through 8th grade. If your child is in 2nd-8th grade they have the opportunity to complete the mySAEBRS survey, which asks them to self-assess using the same 19 questions their educator is answering. The purpose of SAEBRS and mySAEBRS is to identify which SEL skills your child has already learned and which skills they would still need to develop. Once our teachers identify the skills their students still need to learn, they will teach those SEL skills. Our goal is to ensure that your child continually develops the SEL skills they need for lifelong success. We believe that the information gathered with the SAEBRS and mySAEBRS assessment will be beneficial to your child's overall success both inside and outside of the classroom.
We are excited to implement SAEBRS this school year and continue to focus on whole children! If you have any questions, please contact your child's teacher.

Climbing Club
Chute Climbing Club will start the year in room 206. Students will need to sign up and attend our meetings in the classroom before they will be allowed to participate in the gym. Our first meeting will be Tuesday, September 10th.
Debate Club
Calling all middle school students: It’s time for debate! The D65 Debate Program, funded by District 65 and covering lunch and transportation costs, offers a structured environment where students can engage in competitive debate, learn to construct and defend arguments and develop their reasoning abilities. Since 2022, we’ve partnered with Argument Centered Education (ACE), a renowned organization dedicated to advancing debate and critical thinking skills, ensuring that our students receive high-quality instruction and mentorship. The program allows students to debate in teams or individually against teams from across the city of Chicago and Northwest Indiana.
Intellectual property rights is the national debate topic for this school year. D65 students will be given a set of files, along with other materials to prepare and practice for the tournaments (view 2024-2025 schedule).
Use the button below to register by September 14! If you have any questions or need assistance, please email Regina Colquitt, Director of Social Studies, at colquittr@district65.net.
